Important information for 5th grade parents-
The State of Georgia requires that all students entering the 6th grade have a current Georgia Certificate of Immunization on file showing documentation of 2 Varicella vaccines or a history of the Chicken Pox. Contact your child’s private physician or the local Health Dept. to obtain the vaccine and a copy of their Certificate of Immunization. Please don’t wait until it is time to return to school in the Fall to get this taken care of.

ALL MEDICATION MUST BE KEPT IN THE NURSE’S OFFICE. Inhalers may be carried by students but the nurse must be aware that the student has the inhaler and paperwork must be completed by the parent.
